Urgent Wrap Around Care South (UWACS)

UWACS comprises of a team of nurses, paramedics, HCAs and an on-call GP. We can take referrals from the ambulance service, paramedics “at the scene”, Call Before You Convey, JET and any South Cambs GP for any acutely unwell patient needing at-home increased care and support to try and prevent admission to hospital.

If you have triaged your patient and believe they need rapid input and investigations to try and avoid admission please call our team.

Our aim is to triage your patient by telephone within 15 minutes of the referral, with a qualified clinician visiting within 2 hours. We will fully assess the patient and, along with an on-call GP, develop a care plan for their acute episode, undertaking any necessary investigations and following up results. We are able to treat your patient for up to 5 days before discharging them back to their own GP. All consultations are fully documented on SystmOne patient notes and we will communicate with the patient’s own GP team at the start and at the end of care.

Our service is available from 8am to 8pm, 7 days a week.

To use our service your patient will need to be:

  • Over 18 years old
  • Registered with a Cambridgeshire or Peterborough GP surgery in south Cambridgeshire
  • Not presenting with a current mental health crisis

Acceptable referrals for acutely unwell patients to prevent ambulance attendance include:

  • Falls (unless they are unable to get up off the floor without assistance)
  • Deteriorating / complex wounds including dressings (not suturing)
  • Chest infections
  • Urgent diabetes care
  • Suspected urinary tract infections / blocked urinary catheters
  • Any acute exacerbation of existing chronic health
  • Acute illnesses in palliative patients (but not prescriptions of controlled drugs)

Procedures we carry out for acutely unwell patients include:

  • ECGs
  • Wound dressings
  • Urgent venepuncture
  • Flushing of vascular devices and care of blocked drains
