If you or someone you know is in mental health crisis, contact CPFT’s First Response Service by ringing NHS 111, and selecting option 2.

There are many local and national resources offering help and support for mental health. From aged 17 and above you can access the NHS Local Wellbeing Hub that can signpost and offer therapy.  This CPFT website has lots of patient resources and videos that are extremely helpful. You can self-refer – no GP appointment necessary.

For those in work or education there may be resources available to you through these areas, so always ask if you feel able to.

For those not in work but trying to get back into the workplace you can find links to local support through the Keep Your Head website.

Granta can offer appointments if you feel you need one and we can also help you navigate what services may be available to you that might help.

If you are looking to be referred for an ADHD assessment we need some specific information first. We would ask that you complete this self-assessment questionnaire and consider your answers, specifically writing something down about how your difficulties affect your functioning and wellbeing, the history of these difficulties, and any other views you have on the problem.

Further information about ADHD can be found on the ADDA website. ADDA is the world’s largest organization dedicated exclusively to helping adults with ADHD to live better lives. 

If you are looking to be referred for autism (ASD) assessment we will need some specific information in the first instance. We ask that you complete this self-assessment questionnaire and consider your answers, detailing how the difficulties affect your functioning and wellbeing, any past history, and any other views you have about your problem.

Further information about ASD can be found on the Autism Research Centre website. The ARC mission is to contact research and understand autism and to improve the health and wellbeing of autistic people and their families.

If you have concerns about your memory or the memory of a friend or relative get in touch, with the patient’s permission where possible. We can perform some basic memory assessments and decide whether blood tests and onward referrals to see a specialist are needed.
